The document is born during the visit itself — by the time you close it, a draft note is ready:

Visit recording happens only with the patient’s consent; audio never leaves the perimeter of the clinic or the HealthOS cloud.

- Time goes back to the patient. You talk to a person instead of retyping what was said into a form: by the end of the visit what is left is confirming a ready draft rather than typing from scratch.
- Every term links to a fragment of the recording. Reviewing the draft is specific: not “read everything” but “confirm this”.
- Disputed bindings are short questions. The system shows grounded options; you choose in seconds.
- Confirmation is always yours. The draft becomes a note only after your sign-off; the product does not diagnose and does not suggest treatment — it structures what you said.
